This form is to be completed in conjunction with a completed medical/incident report.

Any ministry area needing to submit a request for funds will have to submit this form for review and approval. Pastors Brandon and Kelly will review and approve. If not approved, then the fund request will go on hold. You will need to submit a finance request for each item. If your area needs monthly purchases such as office supplies, please check the reoccurring option below.

Please complete and submit 60-90 days BEFORE requested event date to allow review and processing time. Incomplete proposals that lack requested information will cause delays to timeline, please be sure to give as much detail as possible. You will be notified within 2-3 weeks of the status of your proposal.

Please submit this form 45 days prior to event/meeting of requested space. These dates will need to be approved by not only Pastors Brandon and Kelly, but also Compass Church.
